Is level 4 supposed to be for grade 4?

Which level would you start your child in if they were still learning the foundations of grammar (nouns, verbs, etc.)?

I believe Level 4 is for 4th grade if you have been following the program from the beginning. Someone can correct me on this if I'm wrong but my dd began with FLL and she is going into 4th grade in the fall and I've ordered Level 4 for her.

I would recommend starting at the beginning. It builds on itself. If some of the lessons are too simplistic for an older child, you could combine lessons or skip some but the basics are reviewed well in the First Language Lessons for the Well Traind Mind and I would highly recommend you start there. My dd in 3rd grade knows every definition for every part of speech, the 4 types of sentences, how to diagram most types of sentences, etc. She has also memorized the entire list of prepositions, helping verbs, linking verbs, etc. If we tried to jump into Level 3 before doing FLL for the WTM, I think she might have been overwhelmed with all the lists that needed to be memorized.

I hope this makes sense.

If it helps, most lessons only took us about 10 minutes a day (sometimes less) in that first book so you should be able to go through it pretty quickly.

I believe they purposely put "Levels" as opposed to grades with the understanding that some children may not be working at the same level.

My 8yo ds is presently doing FLL 1/2 doubled-up. (We are using WWE alongside it so we skip the narrations.) We tried FLL when he was in 1st grade and it was too abstract for him in terms of understanding parts of speech. I can honestly say that he is thriving now and and I'm very pleased with his retention.

In your specific case though, I'd consider Level 3 or 4 if your dc is still mastering the parts of speech. You could start with Level 3 and do more than the suggested 3 lessons per week if you felt the need to, or possibly take Level 4 at slower clip. I believe they both have approximately 100 or so lessons.
