View Full Version : Does anyone have a nice template for quarterly reports they can share?

I've been hand-writing dd's because I like the form I have-has boxes so I don't have to write too much inside! But I need to move to doing my forms on the computer; ds hits first grade in the fall and I can't do TWO handwritten forms each quarter.
I don't want to do Word because I was hoping to have boxes. So I thought Excel would be nice, but it doesn't have to be. I am not familiar enough with Excel (that's an understatement :lol:) to make one myself. Even if yours is for a different state than mine, I can modify it to show our state's requirements. Thanks!

I'm in NY. We have to do quarterly reports.
I set up my quarterly much like my IHIP (individualized home instruction plan). I list the subjects. Under each I describe what we plan to do. Usually I just list a book and a few buzz words (topics). Then come quarterly report time I write 2 lines at the top (after the demographic stuff: name, address, etc). I write: " Hours of instruction: 225+ hours of instruction were provided" and " Assessment: CHILD'S NAME is progressing at a satisfactory level or above in all subject matter."
Then under each subject (as listed on the IHIP) I list the topics we covered. So, for example, under spelling I would write: " Spelling: spelling list words covering the following rules: consonant blends sm, sp, st, fr, cr, br, bl, fl, pl, tr, dr, nd, lt; y as a vowel, long o and u, long e; proofreading, word usage, alphabetizing, syllables"
That is all I do. It satisfies the regulations. I state the number of hours, what we did, and an indication of the child's progress.
So for the next quarter, I just open up that file, change the topics covered, and save as a new file. I chose my dates to be evenly spaced based on a year round schedule. So each month I add to the form. It takes a few minutes to add in my info each month. Then come time to submit it is done. For some subjects it doesn't change much.
That works well for me.

just got back, sorry. I will probably end up doing like Wendy, so I don't have to give grades, etc. I had hoped for little boxes so I wouldn't have to write much in the content section, but what Wendy is doing is what I figured I'd have to end up doing. It will definitely make it easier to have it made up on the computer and just type the things in! I already do my IHIPs on the computer.
