For my son's transcript, I used A Garden Patch (http://gardenpatchproducts.com/) . His portfolio turned out REALLY nice.

The cd also provides LOTS of other forms for homeschooling. Look for it on the swap board, or ebay. I think I purchased mine for under $10.

It seems to me that maybe Oak Meadow will allow you to incorporate at least some of your own programming into their transcript program.
