Need spacial type of transcript form

Becuse of my daughter's medical conditions, I do not want a high school transcript with 1st. semester/2nd semester type format. I would rather it be by year and one that would let me include courses she is still doing. This is not a final transcript but I have to have one for dual enrollment and also for doing any athletics. Template that would let me do this would be best. I don't want to include percentage grades but would include a grading scale. Last of all, it must be easy. I have Homeschool tracker Plus and I just don't have the time to learn how to do it.
