Presidents notebook page...

I want my dd to create Presidents notebook, to go along with our history studies.

I am looking for a website that has a printable form for a notebooking "President page." I used to have a site bookmarked that had printable forms for presidents, explorers, etc. but no longer have it marked. Do you know of a good site with forms like this?

Or please share any neat ideas you used for a president's notebook!
