creating a high school transcript

Can someone please direct me to a decent online template if one exists? I found one that was awful and am looking for something more professional looking. I looked into buying Transcript Pro, but dh said he could create one just as good for free. As you may have guessed, dh has not had time to make one for me. I really need to get a transcript done for ds's dual high-school enrollment at the local CC this year.

the covenant college one cited above. It's a little more involved than some I've seen get accepted, but it covers the majority of what was on numerous ones I've looked at. (and I've looked at many).
