I'm working on Homeric Greek with A Reading Course in Homeric Greek (Lesson 17, so I can't really review it yet). For Attic Greek I've read good things about Athenaze and JACT's reading course. I can't help with Koine or modern.

I recently backed up to work on Harvey Bluedorn's A Greek Alphabetarion - I've discovered its almost impossible (for me) to memorize words I can't pronounce. Bluedorn has a high-school level course called Homeschool Greek, and he claims to emphasize reading, which I think is important.
