What information should be included on the transcript (besides classes, grades, credits...)?

Is it better to list the classes out by year or by class?

I thought I would just give a grade to academic classes, not things like PE, art, music, etc. Is that a good idea? I would still give credit, though.

Is it necessary to have a name for your home school or can you just say "official transcript of (your student)"?

Thanks for answering these questions. My dd is applying to the cc for classes next fall and they need to see a transcript to show she is a junior.
