They will probably have a specific form. You can call and ask if they will accept a general form, but most will not.

I called my new doc and they just said to type a generic, do you think I should call my old doctor?

I would call the old office, they may (not sure) need you to sign a release form of some kind.

The old doctor is the one who will need the authorization to release the records.


If not, have you tried googling for "blank printable medical release form" or something similar?
