I would think Rainbow Resource would have it. I think Amazon has it.

Here's the Rainbow Resource link (http://www.rainbowresource.com/product/Format+Writing/013488/1249271200-1810273); the information there says it retails for $22.00 but that RR sells it for $19.50.

Check out homeschoolclassifieds to find it used. I just sold mine for $13 ppd there.
